| 目的 探讨温针灸结合耳穴贴压治疗寒凝湿滞型肩周炎的临床疗效及安全性。方法 按随机数字表法将我院2020年5月至2022年5月收治的64例寒凝湿滞型肩周炎患者分入两组。对照组32例给予双氯芬酸钠缓释片治疗，治疗组32例在对照组的基础上采取温针灸结合耳穴贴压治疗。对比两组治疗前后中医症候积分、Constant-Murley量表评分、血清前列腺素E2（PGE2）、肿瘤坏死因子-α（TNF-α）水平，分析两组临床疗效与不良反应。结果 与对照组比较，治疗组总有效率更高（96.88％vs 78.13％）（χ2=5.143，P=0.023）；治疗后2组中医症候评分均降低，且相比对照组，治疗组关节疼痛、关节畏寒、关节畏风、关节沉重评分更低（P<0.05）；治疗后2组Constant-Murley量表各项评分升高，且相比对照组，治疗组治疗后疼痛、关节活动度、肌力、日常生活评分更高（P<0.05）；治疗后2组血清PGE2、TNF-α水平均降低，且治疗组治疗后血清PGE2、TNF-α水平低于对照组（P<0.05）；治疗组不良反应发生率6.25％，对照组为9.38％，两组比较无统计学差异（χ2=0.217，P=0.641）。结论 温针灸结合耳穴贴压治疗寒凝湿滞型肩周炎患者能取得较好的临床疗效，不但能改善患者临床症状和日常生活能力，抑制炎症反应，还可提升患者肌力和肩关节活动功能，具有安全性，值得推广。 |

| Objective: To explore the effect and safety of warm acupuncture combined with auricular point sticking in the treatment of periarthritis of shoulder with cold coagulation and wet stagnation. Methods: By the random number table method, 64 cases with frozen and wet stagnant periarthritis of shoulder admitted to our hospital from May 2020 to May 2022 were divided into two groups. 32 patients in the control group were treated with diclofenac sodium sustained-release tablets, and 32 patients in the treatment group were treated with warm acupuncture combined with auricular point sticking. TCM symptoms, Constant-Murley scale scores, serum PGE2 and TNF-α before and after treatment were compared between the two groups, and the clinical efficacy and side reactions were analyzed. Results: The effective rate of the treatment group was higher when compared with control group(96.88% vs 78.13%) (χ2=5.143, P=0.023). After treatment, TCM symptom scores in two groups were decreased, and the scores of joint pain, joint chills, joint wind and joint heaviness of the treatment group were lower when compared with control group(P<0.05). After treatment, the scores of Constant-Murley scale were increased in both groups, and the scores of pain, range of motion, muscle strength and daily life in the treatment group were higher when compared with the control group(P<0.05). After treatment, the levels of PGE2 and TNF-α in the two groups were decreased, and the levels of PGE2 and TNF-α in the treatment group were lower when compared with control group(P<0.05). The incidence of side reactions was 6.25% in treatment group and 9.38% in control group, with no significant difference(χ2=0.217, P=0.641). Conclusion: Warm acupuncture and moxibustion combined with auricular point sticking in the treatment of patients with cold coagulation and wet stagnant periarthritis of shoulder has definite curative effect, which can relieve pain, improve patients' clinical symptoms and daily living ability, inhibit inflammatory response, improve patients' muscle strength and shoulder joint activity function. It is safe and worth promoting. |
