| 【】目的 探讨杭州市富阳区关于影响慢性阻塞性肺疾病(COPD)的相关影响因素,并因此构建出具体的筛查模型。方法? 随机选择2022年05月至2024年02月在本院医共体区域范围内疑诊为慢性阻塞性肺疾病患者4027例,根据肺功能结果分为COPD组和对照组,通过单因素分析和多因素Logistic回归分析得出独立影响因素,建立COPD筛查模型并绘制ROC曲线。结果? 单因素统计分析显示,COPD组患者的性别、慢性支气管炎疾病史、高血压疾病史、糖尿病史、吸烟情况、经常咳痰、有过喘息发作、运动后喘息或咳嗽、因呼吸道症状就医、氧饱和度、mMRC等级、CAT评分比较差异在统计学上均有意义(均P<0.05),其中多因素Logistic回归统计分析表明,性别、慢性支气管炎疾病史、糖尿病疾病史、吸烟情况、因呼吸道症状就医、氧饱和度、mMRC等级、CAT评分是COPD的发生的独立影响因素(P<0.05),并对其赋分,计算得分,绘制ROC曲线:AUC=0.617,95%CI:0.595~0.639(P<0.05),最佳临界值为70.5分,诊断界值下模型的敏感度、特异性、阴性预测值、阳性预测值分别为71.9%、47.2%、88.5%、22.8%。结论? 性别、慢性支气管炎史、糖尿病史、吸烟情况、因呼吸道症状就医、氧饱和度、mMRC等级、CAT评分对慢性阻塞性肺疾病患者具有较高的预测价值,本研究构建的筛查模型预测能力较好,具有较高的灵敏度和阴性预测值。 |

| 【】Objective To explore the related influencing factors of chronic obstructive pulmonary disease ( COPD ) in Fuyang District of Hangzhou, and to construct a specific screening model. Methods 4027 patients with suspected COPD who were diagnosed in the medical community area of our hospital from May 2022 to February 2024 were randomly selected. According to the results of lung function, they were divided into COPD group and control group. Independent influencing factors were obtained by univariate analysis and multivariate Logistic regression analysis, and the screening model of COPD was established and ROC curve was drawn. |
