| 【摘要】 目的 本研究旨在评估胸腔积液中游离结核分枝杆菌DNA （MTB cfDNA）对结核性胸膜炎的诊断价值。方法 收集2022年4月至2022年11月杭州市红十字会医院疑似结核性胸膜炎住院患者的胸腔积液样本，进行Xpert MTB/RIF（Xpert）、分枝杆菌培养和MTB cfDNA检测。以临床诊断为标准，分析MTB cfDNA的诊断效能。结果 最终纳入115名患者，包括 92例临床诊断的结核性胸膜炎患者和23例非结核性胸膜炎患者。MTB cfDNA的检测敏感性为51.1%（47/92），优于Xpert 20.7%（19/92）和培养10.9%（10/92），差异均有统计学意义（P<0.001）。三种检测方法的特异性均为100%。MTB cfDNA对结核性胸膜炎的诊断AUC为0.755，高于培养0.554和Xpert 0.603，差异均有统计学意义(P< 0.05)。结论 胸腔积液MTB cfDNA检测对结核性胸膜炎有良好的诊断价值。 |

| 【Abstract】 Objective The aim of this study was to evaluate the diagnostic value of free Mycobacterium tuberculosis DNA (MTB cfDNA) in pleural effusion for tuberculous pleurisy. Methods Pleural effusion samples from patients admitted to Hangzhou Red Cross Hospital with suspected tuberculous pleurisy from April 2022 to November 2022 were collected for Xpert MTB/Rif (Xpert) , mycobacterial culture, and MTB cfDNA testing. The diagnostic efficacy of MTB cfDNA was analyzed according to clinical diagnosis. Results Ultimately, 115 patients were included, including 92 clinically diagnosed tuberculous pleurisy patients and 23 non-tuberculous pleurisy patients . The sensitivity of MTB cfDNA was 51.1% (47/92) , which was superior to that of Xpert 20.7% (19/92) and culture 10.9% (10.92) . The difference was statistically significant (P<0.001) . The specificity of all three assays was 100% . The AUC of MTB cfDNA for the diagnosis of tuberculous pleurisy was 0.755, higher than that of culture 0.554 and Xpert 0.603, and the difference was statistically significant (P< 0.05). Conclusion Detection of MTB cfDNA in pleural fluid demonstrates good diagnostic value for tuberculous pleurisy. |
