| 目的 本研究探讨了新组中药外洗方配合负压灌洗联合他达拉非治疗血管性勃起功能障碍患者的临床疗效。方法 选择2023年1月—2023年8月在男科门诊就诊的87例血管性ED患者分为观察组43例和对照组44例。观察组采用中药负压灌洗联合口服他达拉非治疗4周,对照组口服他达拉非,连服4周。观察治疗前后患者IIEF-5、EHS、GAD评分以及PSV、EDV、RI等血流动力学指标的变化。结果 观察组治疗后IIEF-5、EHS、GAD评分以及PSV、EDV、RI较治疗前有所改善(P＜0.05),与对照组相比效果更为显著(P＜0.05)。观察组中有动脉性勃起功能障碍21例,静脉性勃起功能障碍22例,治疗后动脉性ED中动脉性指标PSV改善明显(P＜0.05),但EDV降低不明显。在静脉性ED患者中PSV升高,EDV下降明显(P＜0.05)。结论 中药负压灌洗联合他达拉非治疗血管性勃起功能障碍可提高阴茎动脉供血,舒张平滑肌,改善静脉血管的关闭机制,从而有效改善患者的勃起功能。 |

| Objective This study discussed the clinical effect of the new group of traditional Chinese medicine external washing prescription combined with negative pressure irrigation and tadalafil in the treatment of vascular erectile dysfunction patients. Methods From January 2023 to December 2023, 87 patients with vascular DE in male outpatient department were selected and divided into observation group (43 cases) and control group (44 cases). The observation group was given TCM negative pressure irrigation combined with oral tadalafil, and the control group was given oral tadalafil for a total of 4 weeks. Changes in IEF-5, EHS, GAD score, PSV, EDV, RI and other hemodynamic indicators were observed before and after treatment. Results After treatment, IIEF-5, EHS, GAD scores, PSV, EDV and RI were improved compared with those before treatment (P < 0.05), and the improvement in observation group was more obvious than that in control group. In the observation group, there were 21 cases of arterial erectile dysfunction and 22 cases of venous erectile dysfunction. After treatment, EHS, IIEF and GAD scores were improved in both groups (P < 0.05). After treatment, the arterial index PSV improved significantly (P<0.05), but EDV did not decrease significantly. PSV increased in patients with intravenous ED (P<0.05), EDV decreased significantly (P<0.05). Conclusion Negative pressure lavage combined with tadalafil in the treatment of vascular erectile dysfunction improves blood supply to the penile arteries, relaxes smooth muscles and improves the mechanism of venous vessel closure, thus effectively improving erectile function in patients. |
