      目的 观察微动力负压敷料治疗深II度烧伤创面的临床疗效。方法 于2021年1月至2023年6月,纳入中国人民解放军联勤保障部队烧伤整形科收治的烧伤面积为1%~20%体表总面积(TBSA)的深II度烧伤患者84例,随机分为试验组(n=42)和对照组(n=42)。试验组使用微动力负压敷料换药治疗,对照组使用磺胺嘧啶银乳膏换药治疗。记录两组患者的创面愈合时间、创面愈合率、手术植皮率、换药次数、换药疼痛评分、二次创伤评分、创面细菌培养情况以及不良反应。结果 试验组创面愈合时间[(21.38?4.80)d]较对照组[(28.60?4.35)d]显著缩短(P<0.01)。伤后14、21、及28d试验组创面愈合率[(48.35?16.38)%、(84.78?14.91)%、(97.73?5.50)%]均明显高于对照组[(41.76?7.98)%、(67.67?9.31)%、(88.64?11.10)%,P值均<0.05]。试验组的手术植皮率(4.76%)低于对照组(28.57%,P<0.01)。试验组创面换药次数(7.10?1.97)次、换药疼痛评分(3.29?1.09)分、换药二次创伤评分(1.04?0.34)均明显少于对照组[(11.45?2.63)次、(4.74?1.23)分、(1.77?0.65)分,P值均<0.01]。伤后7、14d试验组创面细菌检出率(23.81%、14.29%)明显低于对照组(47.61%、35.71%,P值均<0.01)。两组患者治疗过程中均无不良反应。结论 微动力负压敷料可以明显缩短深II度烧伤创面的愈合时间,降低植皮手术率和创面感染率,提高换药过程的舒适度。

      ABSTRACT Objective To preliminarily observe the effects of application of micro-dynamic negative pressure dressing in treatment of deep partial-thickness burn wounds. Method From January 2021 to June 2022, 84 patients with deep partial-thickness burn wounds on 1% to 20% of the total body surface area(TBSA) who are admitted to Department of Burn and Plastic Surgery of 903rd Hospital of PLA were recruited in this prospective randomized controlled study. According to the random number table, they were divided into treatment(n=42) and control groups(n=42). The micro-dynamic negative pressure dressing was used for the treatment group, and the silver sulfadiazine cream was used for the control group. The complete wound healing time, the rate of wound healing, the rate of surgical grafting, the frequency of dressing change, the pain score and the secondary trauma score during the dressing change, the bacterial culture of wound and the adverse reaction were recorded in patients of 2 groups. Result The complete wound healing time of patients in the treatment group[(21.38?4.80)d] was significantly shorted than the control group[(28.60?4.35)d, P<0.01]. On PID 14,21 and 28, the wound healing rates of patients in the treatment group[(48.35?16.38)%,(84.78?14.91)%,(97.73?5.50)%] were significantly higher than the control group[(41.76?7.98)%,(67.67?9.31)%,(88.64?11.10)%, P <0.05]. The rate of surgical grafting of patients in the treatment group(4.76%)was significantly less than the control group(28.57%,P <0.01). The frequency of dressing change(7.10?1.97), the pain score (3.29?1.09)and the secondary trauma score(1.04?0.3)during the dressing change of patients in the treatment group were lower than the control group[(11.45?2.63),(4.74?1.23),(1.77?0.65),P<0.01]. On PID 7and 14, the detection yields of bacteria on wound in the treatment group(23.81%、14.29%)were significantly lower than the control group(47.61%,35.71%,P <0.01). There were no evident adverse reaction in the two groups of patients. Conclusion The treatment of application of micro-dynamic negative pressure wound dressing in deep partial-thickness burn wounds can shorten the healing time significantly, decrease the rate of surgical skin grafting and the wound infection, and improve the comfort of at changing dressings.
