      摘要:目的 分析血清磷脂酶A2(PLA2)、磷酸甘油酸激酶1(PGK1)对老年前列腺癌根治性手术疗效的预测价值。方法 以2019年10月~2022年10月拟于本院行前列腺癌根治性手术的78例老年前列腺癌患者为研究对象,依据其手术疗效将其分为有效组(n=50)和无效组(n=28)。所有研究对象均在术前进行血清PLA2、PGK1、前列腺特异性抗原(PSA)水平检测,采用Pearson相关性分析血清术前PLA2、PGK1水平与术后PSA水平的相关性。采用二元Logistic回归分析法构建以PLA2、PGK1为变量的预测模型,以受试者特征工作曲线(ROC)评价血清PLA2、PGK1水平对老年前列腺癌患者根治性手术疗效的预测价值。结果 无效组的血清PLA2、PGK1、PSA水平高于有效组(P<0.05);血清PLA2、PGK1水平与PSA水平呈正相关(r=0.242,0.263,P<0.05)。Logistic回归分析结果显示,以PLA2、PGK1构建的预测模型为Y=-6.752+0.106×PLA2+0.232×PGK1。ROC曲线分析显示,血清PLA2、PGK1联合预测老年前列腺癌患者根治性手术疗效的曲线下面积(AUC)为0.924,灵敏度为96.43%,特异性为88.00%,且二者联合预测的效能优于各指标单独预测(P<0.05)。结论 血清PLA2、PGK1水平与老年前列腺癌患者根治性手术疗效密切相关,通过术前测定血清PLA2、PGK1水平可为根治性手术效果评估及术后辅助治疗方案的选择提供一定的指导价值。

      Objective To analyze the predictive value of serum phospholipase A2 (PLA2) and phosphoglycerate kinase 1 (PGK1) on the efficacy of radical surgery in elderly patients with prostate cancer (PCa). Methods A total of 78 elderly patients with PCa who were scheduled to undergo radical surgery for prostate cancer in the hospital were enrolled in the study from October 2019 to October 2022. According to the surgical efficacy, the patients were divided into effective group (n=50) and ineffective group (n=28). The levels of serum PLA2, PGK1 and prostate specific antigen (PSA) were detected in all study subjects before surgery. Pearson correlation analysis was used to analyze the correlation between serum PLA2 and PGK1 levels before surgery and PSA levels after surgery. Binary Logistic regression analysis was adopted to construct the prediction model with PLA2 and PGK1 as variables. Receiver operating characteristic curve (ROC) was applied to evaluate the predictive value of serum PLA2 and PGK1 levels on the efficacy of radical surgery in elderly patients with PCa. Results The levels of serum PLA2, PGK1 and PSA in the ineffective group were higher than those in the effective group (P<0.05). Serum PLA2 and PGK1 levels were positively correlated with PSA level (r=0.242, 0.263, P<0.05). Logistic regression analysis indicated that the prediction model constructed with PLA2 and PGK1 was Y=-6.752+0.106×PLA2+0.232×PGK1. ROC curve analysis revealed that the area under the curve (AUC), sensitivity and specificity of the combination of serum PLA2 and PGK1 in predicting the efficacy of radical surgery in elderly patients with PCa were 0.924, 96.43% and 88.00%, and the efficiency of the combined prediction was better than that of each index alone (P<0.05). Conclusion Serum PLA2 and PGK1 levels are closely related to the efficacy of radical surgery in elderly patients with PCa. Preoperative determination of serum PLA2 and PGK1 levels can provide certain guiding value on the effect evaluation of radical surgery and the selection of postoperative adjuvant therapy.
