| 目的 探讨精准内口处理联合拖线垫棉疗法治疗高位肛周脓肿的临床疗效。方法 收集镇海区中医医院高位肛周脓肿患者90例，随机分配到治疗组和对照组，每组包含45例。治疗组采用精准内口处理联合拖线垫棉疗法治疗；对照组采用传统的切开排脓治疗,观察比较两组的临床疗效、创面疼痛VSA评分、创面分泌物、创面愈合时间、住院时间和肛门功能情况。结果 治疗组的总有效率93.3%（42/45），对照组为71.2%（32/45），治疗组明显高于对照组（P＜0.05）；治疗组术后第1、3、7天的VSA评分均低于对照组（P＜0.05）；治疗组术后第3、7、14天的创面分泌物均少于对照组（P＜0.05）；创面愈合时间，住院时间均短于对照组（P＜0.05），两组术后第14天的VSA评分、术后第1天创面分泌物、术前肛门功能评分和术后6月肛门功能评分方面比较，结果未显示出统计学上的显著性（P＞0.05），治疗组术后复发率明显低于对照组（P＜0.05）。结论 精准内口处理联合拖线垫棉疗法治疗高位肛周脓肿可以减轻术后疼痛、减少术后创面分泌物，缩短住院时间和创面愈合时间，降低术后复发率。 |

| Objective：To evaluate the clinical efficacy of precise internal mouth treatment combined with thread dragging cotton pad therapy in the treatment of high perianal abscess. Methods：Ninety patients with high perianal abscess admitted to the Zhenhai District Traditional Chinese Medicine hospital were randomized into the treatment group and the control group, each with 45 cases. Those in the treatment group received precise internal mouth treatment combined with thread dragging cotton pad therapy. Several clinical outcomes, including clinical efficacy, Postoperative Visual Analogue Scale (VSA) score of wound pain, wound secretions, wound healing time, hospital stay and anal function were compared between the two groups. Results：The total effective rate for the treatment group was 93.3% (42/45 patients), which was significantly higher compared with 71.2% (32/45 patients of the control group; P<0.05. The VAS scores on days 1, 3, and 7 were significantly lower in the treatment group than in the control group (P<0.05). The wound secretion on days 3, 7, and 14 post-surgery was lower in the treatment group compared with the control group (P<0.05). Moreover, the wound healing time and hospital stay duration were significantly shorter in the treatment group than in the control group (P<0.05). However, no statistically significant differences were observed between the two groups in the VAS score on the 14th postoperative day, wound secretion on the 1st postoperative day, preoperative anal function score, or the anal function score at the 6-month follow-up (P>0.05). Overall, the recurrence rate in the treatment group was significantly lower than that of the control group (P<0.05). Conclusion：Precise internal mouth treatment combined with thread dragging cotton pad therapy reduced postoperative pain, postoperative wound secretions, shortened the hospital stay and wound healing time, and reduced postoperative recurrence rate in patient with high perianal abscess. |
