      【】 目的 研究心电图最深S波、QT间期离散度、Selvester QRS心电图评分对高血压患者左心室肥厚的诊断价值及其与心功能的关系。方法 我院2021年3月~2024年1月收治的高血压患者264例根据患者是否发生左心室肥厚分为左心室肥厚组(151例)和左心室正常组(113例),比较两组心功能、心电图指标,分析心电图指标单独、联合对高血压患者左心室肥厚的诊断价值及心电图指标与心功能的相关性。结果 左心室肥厚组左室后壁厚度(LVPWT)、室间隔厚度(IVS)、左心室舒张末期内径(LVEDd)、左室质量指数(LVMI)、室间隔厚度(IVST)及最深S波、QT间期离散度、Selvester QRS心电图评分高于左心室正常组(P<0.05)。最深S波、QT间期离散度、Selvester QRS心电图评分联合诊断高血压患者左心室肥厚的曲线下面积(AUC)值高于三者单独检测(P<0.05)。高血压左心室肥厚患者最深S波、QT间期离散度、Selvester QRS心电图评分与LVPWT、IVS、LVEDd、LVMI、IVST均呈正相关关系(r=0.622、0.548、0.615、0.538、0.619、0.547、0.572、0.504、0.604、0.496、0.463、0.482、0.681、0.559、0.531,P<0.05)。结论 高血压患者左心室肥厚的发生可引起患者心电图最深S波、QT间期离散度、Selvester QRS心电图评分升高及心功能指标改变,心电图最深S波、QT间期离散度、Selvester QRS心电图评分联合可有效提高对高血压患者左心室肥厚的诊断价值,同时三者与患者心功能呈正相关关系。

      Objective To study the diagnostic value of the deepest S wave of electrocardiogram, QT interval dispersion and score of Selvester QRS electrocardiogram for left ventricular hypertrophy in patients with hypertension and its relationship with cardiac function. Methods A total of 264 hypertensive patients admitted to our hospital from March 2021 to January 2024 were divided into left ventricular hypertrophy group (151 cases) and left ventricular normal group (113 cases) according to whether the patients had left ventricular hypertrophy or not. The cardiac function and electrocardiogram indexes of the two groups were compared, and the diagnostic value of electrocardiogram indexes alone and combined on left ventricular hypertrophy in hypertensive patients and the correlation between electrocardiogram indexes and cardiac function were analyzed. Results The left ventricular posterior wall thickness (LVPWT), interventricular septal thickness (IVS), left ventricular end-diastolic diameter (LVEDd), left ventricular mass index (LVMI), interventricular septal thickness (IVST), deepest S wave, QT interval dispersion and score of Selvester QRS electrocardiogram in the left ventricular hypertrophy group were higher than those in the left ventricular normal group (P<0.05). The area under the curve (AUC) value of the combination of the deepest S wave, QT interval dispersion and score of Selvester QRS electrocardiogram in the diagnosis of left ventricular hypertrophy in hypertensive patients was higher than that of the three separate tests (P<0.05). The deepest S wave, QT interval dispersion and score of Selvester QRS electrocardiogram were positively correlated with LVPWT, IVS, LVEDd, LVMI and IVST in patients with hypertensive left ventricular hypertrophy (r=0.622, 0.548, 0.615, 0.538, 0.619, 0.547, 0.572, 0.504, 0.604, 0.496, 0.463, 0.482, 0.681, 0.559, 0.531, P<0.05). Conclusion The occurrence of left ventricular hypertrophy in hypertensive patients could cause the deepest S wave of electrocardiogram, QT interval dispersion, score of Selvester QRS electrocardiogram and cardiac function index changes. The combination of the deepest S wave of electrocardiogram, QT interval dispersion and score of Selvester QRS electrocardiogram could effectively improve the diagnosis of left ventricular hypertrophy in hypertensive patients. At the same time, the three are positively correlated with the cardiac function of patients.
