| 目的 研究肝郁气滞型胃溃疡患者经柴胡舒肝散联合艾司奥美拉唑治疗的效果。方法 前瞻性选取2021年10月至2024年2月我院收治的120例胃溃疡患者,根据治疗方案分为对照组、联合组,各60例。对照组采用艾司奥美拉唑治疗,联合组采用柴胡舒肝散联合艾司奥美拉唑治疗。比较两组临床疗效、治疗前后中医证候积分(胃脘胀闷、脘痛连胁、反酸、嗳气)、血清胃肠激素［胃动素(MTL)、胃泌素(GAS)］水平、血清转化生长因子-β1(TGF-β1)、白细胞介素-6(IL-6)、表皮生长因子受体(EGFR)、细胞外调节蛋白激酶1/2(ERK1/2)水平、不良反应发生率。结果 联合组临床总有效率(96.67%比86.67%)高于对照组(P＜0.05)；治疗后,联合组反酸、脘痛连胁、嗳气、胃脘胀闷积分低于对照组(P＜0.05)；治疗后,与对照组相比,联合组血清GAS水平较低,MTL水平较高(P＜0.05)；与对照组相比,联合组治疗后血清TGF-β1、IL-6水平较低,ERK1/2、EGFR水平较高(P＜0.05)；两组间不良反应总发生率比较,无显著差异(P＞0.05)。结论 采用柴胡舒肝散联合艾司奥美拉唑治疗可有效缓解肝郁气滞型胃溃疡患者临床症状及减轻炎症,调节胃肠激素水平,提高疗效,且安全性良好。 |

| Objective To investigate the therapeutic effect of Chaihu Shugan Powder combined with esomeprazole in treating gastric ulcer patients with liver-qi stagnation. Methods A prospective study was conducted on 120 patients with gastric ulcer admitted to our hospital from October 2021 to February 2024. These patients were divided into a control group and a combined group, with 60 cases in each group. The control group received esomeprazole treatment, while the combined group received combined treatment with Chaihu Shugan Powder and esomeprazole. The clinical efficacy and incidence of adverse reactions were compared between the two groups, as well as the scores of traditional Chinese medicine symptoms (stomach distension and stuffiness, abdominal pain radiating to the flank, acid reflux, and belching), serum gastrointestinal hormone levels [motilin (MTL), gastrin (GAS)], serum transforming growth factor-β1 (TGF-β1), interleukin-6 (IL-6), epidermal growth factor receptor (EGFR), and extracellular regulated kinase 1/2 (ERK1/2) levels before and after treatment. Results The total effective rate of the combined group (96.67% vs 86.67%) was higher than that of the control group (P<0.05); after treatment, the scores of acid regurgitation, epigastric pain and hypochondria, belching and epigastric distension in the combined group were lower than those in the control group(P<0.05); after treatment, compared with the control group, the serum GAS level in the combination group was lower, the MTL level was higher(P<0.05); compared with the control group, the levels of serum TGF-β1 and IL-6 were lower, the levels of ERK1/2 and EGFR were higher (P<0.05); there was no significant difference in the total incidence of adverse reactions between the two groups (P>0.05). Conclusion The treatment of Chaihu Shugan Powder combined with esomeprazole can effectively alleviate the clinical symptoms and reduce inflammation in patients with gastric ulcer of liver depression and qi stagnation type, regulate the levels of gastrointestinal hormones, improve the efficacy, and has good safety. |
