| 目的 通过前瞻性研究观察龙胆泻肝汤辅助外用糖皮质激素降阶梯疗法治疗阴囊湿疹湿热浸淫证患者的临床效果。方法? 将安吉县中医医院皮肤科收治的阴囊湿疹湿热浸淫证患者共84例按随机数字表法分为观察组与对照组(各42例),观察组剔除1例,对照组剔除1例,最终2组完成研究者各为41例。观察组给予龙胆泻肝汤辅助外用糖皮质激素降阶梯疗法治疗,对照组仅给予外用糖皮质激素降阶梯疗法治疗,均连续治疗3周后评估2组临床疗效、中医证候积分、阴囊湿疹皮损及严重程度指数(EASI)评分、药物不良反应,并于疗程结束后的3个月、6个月进行期随访以统计复发情况。结果? 治疗3周后,观察组总有效率92.68%(38/41)高于对照组75.61%(31/41)(P＜0.05)；2组湿热浸淫证候积分(主症、次症及总积分)均低于治疗前(P＜0.05),观察组主症积分、次症积分及总积分均低于对照组(P＜0.05)；2组EASI量表各项评分均低于治疗前(P＜0.05),观察组EASI量表各项评分均低于对照组(P＜0.05)；用药期间,观察组不良反应发生率12.20%(5/41)低于对照组29.27%(12/41)(P＜0.05)；疗程结束后经随访,观察组治疗后3个月复发率5.88%(1/17)、6个月复发率11.76%(2/17)均分别低于对照组的40.00%(3/10)、60.00%(6/10)(P＜0.05)。结论? 龙胆泻肝汤辅助外用糖皮质激素降阶梯疗法治疗阴囊湿疹湿热浸淫证患者疗效确切,在缓解症状、减轻皮损面积及瘙痒程度、降低不良反应及复发率方面具有积极作用,值得临床借鉴。 |

| Objective: To observe the clinical effect of Longdan Xiegan Tang as an adjuvant therapy with topical corticosteroids for the treatment of patients with scrotal eczema and damp heat immersion syndrome through prospective research. Methods: A total of 84 patients with scrotal eczema and damp heat immersion syndrome admitted to the Dermatology Department of Anji County Traditional Chinese Medicine Hospital were randomly divided into an observation group and a control group (42 cases each) using a random number table method. One case was excluded from the observation group and one case was excluded from the control group. The final two groups had 41 participants each. The observation group was treated with Longdan Xiegan Tang as an adjunct to topical corticosteroid ladder lowering therapy, while the control group was only treated with topical corticosteroid ladder lowering therapy. After continuous treatment for 3 weeks, the clinical efficacy, TCM syndrome score, scrotal eczema lesion and severity index (EASI) score, and adverse drug reactions of both groups were evaluated. Follow up was conducted at 3 and 6 months after the end of the treatment to record recurrence. After 3 weeks of treatment, the total effective rate of the observation group was 92.68% (38/41), which was higher than the control group"s 75.61% (31/41) (P<0.05); The scores of the two groups of damp heat immersion syndrome (main symptom, secondary symptom, and total score) were lower than before treatment (P<0.05), and the main symptom score, secondary symptom score, and total score of the observation group were lower than those of the control group (P<0.05); The scores of all items on the EASI scale in both groups were lower than before treatment (P<0.05), and the scores of all items on the EASI scale in the observation group were lower than those in the control group (P<0.05); During the medication period, the incidence of adverse reactions in the observation group was 12.20% (5/41) lower than that in the control group (29.27% (12/41)) (P<0.05); After follow-up after the treatment, the observation group had a recurrence rate of 5.88% (1/17) at 3 months and 11.76% (2/17) at 6 months, which were lower than the control group"s 40.00% (3/10) and 60.00% (6/10), respectively (P<0.05). Conclusion: The use of Longdan Xiegan Tang in conjunction with topical corticosteroid step-down therapy is effective in treating patients with scrotal eczema and damp heat immersion syndrome. It has a positive effect in relieving symptoms, reducing skin lesion area and itching, lowering adverse reactions and recurrence rates, and is worthy of clinical reference. |
