| 目的:探究彩色多普勒超声结合血清纤溶酶原激活物抑制剂-1(PAI-1)、肌动蛋白凝胶蛋白2(TAGLN2)在乳腺癌早期诊断中的临床价值。方法:选取2023年02月-2024年11月本院收治的乳腺癌患者106例(研究组)与乳腺良性肿瘤患者100例(对照组),Kappa检验分析彩色多普勒超声、血清PAI-1、TAGLN2对乳腺癌诊断效能;Logistics分析影响乳腺癌发生的因素;ROC分析彩色多普勒超声、血清PAI-1、TAGLN2对乳腺癌的诊断价值;Delong检验比较AUC。结果:与对照组相比,研究组血清PAI-1、TAGLN2显著升高,差异具有统计学意义(P<0.05);研究组肿瘤形态不规则、边界模糊、边缘毛刺、后方回声衰减、微小钙化、纵横比≥1、血流分级Ⅱ-Ⅲ占比高于对照组,差异具有统计学意义(P<0.05);三者联合对乳腺癌早期诊断效能优于单独诊断;肿瘤形态、边界、边缘、后方回声、微小钙化、纵横比、血流分级、PAI-1、TAGLN2是乳腺癌发生的危险因素(P<0.05);彩色多普勒超声、血清PAI-1、TAGLN2三者联合早期诊断乳腺癌的敏感度87.74%,特异度88.00%、AUC是0.944(95%CI=0.915-0.972),三者联合早期诊断乳腺癌的价值高于单独诊断(Z三者联合-彩色多普勒超声=6.292、P<0.001,Z三者联合-PAI-1=5.881、P<0.001,Z三者联合-TAGLN2=5.400、P<0.001)。结论:乳腺癌患者彩色多普勒超声及血清血清PAI-1、TAGLN2发生变化,彩色多普勒超声结合血清PAI-1、TAGLN2对乳腺癌患者早期诊断有一定参考价值。 |

| Objective: To explore the clinical value of color Doppler ultrasound combined with serum plasminogen activator inhibitor-1 (PAI-1) and transgelin 2 (TAGLN2) in the early diagnosis of breast cancer. Methods: From February 2023 to November 2024, 106 patients with breast cancer (research group) and 100 patients with benign breast tumors (control group) in our hospital were gathered. Kappa test was used to analyze the diagnostic efficacy of color Doppler ultrasound, serum PAI-1 and TAGLN2 in breast cancer. Logistic method was used to analyze the factors affecting the occurrence of breast cancer. ROC was used to analyze the diagnostic value of color Doppler ultrasound, serum PAI-1 and TAGLN2 in breast cancer. In addition, Delong test was used to compare AUC. Results: Compared with control group, serum PAI-1 and TAGLN2 in research group were obviously higher (P<0.05). The proportions of tumors with irregular morphology, blurred boundaries, spiculated edges, posterior echo attenuation, microcalcifications, aspect ratio ≥ 1, and blood flow grade II-III in research group were higher than those in control group (P<0.05). The efficacy of combination of three methods in early diagnosis of breast cancer was better than single diagnosis. The risk factors of breast cancer included tumor shape, border, edge, posterior echo, microcalcification, aspect ratio, blood flow grade, PAI-1, TAGLN2 (P<0.05). The sensitivity and specificity of the combination of color Doppler ultrasound, serum PAI-1 and TAGLN2 in early diagnosis of breast cancer were 87.74%, 88.00%, and the AUC was 0.944 (95% CI=0.915-0.972). The value of combination of three factors in early diagnosis of breast cancer was higher than that of single diagnosis (Z combination-color Doppler ultrasound=6.292, P<0.001, Z combination-PAI-1=5.881, P<0.001, Z combination-TAGLN2=5.400, P<0.001). Conclusion: Color Doppler ultrasound and serum PAI-1, TAGLN2 change in breast cancer patients. Color Doppler ultrasound combined with serum PAI-1, TAGLN2 has certain reference value for early diagnosis of breast cancer patients. |
