      目的 探究虎杖苷(polydatin,PD)纳米水凝胶治疗骨折术后慢性难愈性创面(chronic refractory wounds,CRW)的疗效。方法 选取60例CRW患者,随机分为研究组(n=30例)和对照组(n=30例)。研究组使用PD纳米水凝胶进行换药,对照组使用凡士林纱布进行换药,疗程28天。观察两组创面细菌培养情况、创面愈合情况、血清炎症因子(IL-6、TNF-α)水平和创面生长因子(VEGF、TGF-β)水平。结果 两组创面菌种分布比较无明显差异(P>0.05);研究组的创面愈合率高于对照组,差异具有统计学意义(P<0.05);两组患者相比于治疗前,IL-6、TNF-α水平明显下降,VEGF、TGF-β水平明显上升,差异具有统计学意义(P<0.05),且研究组改善效果明显优于对照组。结论 PD纳米水凝胶治疗CRW疗效显著,可以有效消除炎症反应,改善生长因子水平,促进创面愈合,且安全性好,然而抑制细菌生长能力有待进一步研究验证。

      Objective To explore the efficacy of polydatin (PD) nano hydrogel in the treatment of chronic refractory wounds (CRW) after fracture surgery. Method Select 60 CRW patients and randomly divide them into a study group (n=30) and a control group (n=30). The study group used PD nano hydrogel for dressing change, and the control group used vaseline gauze for dressing change, with a course of treatment of 28 days. Observe the bacterial culture, wound healing, serum inflammatory factor (IL-6, TNF - α) levels, and wound growth factor (VEGF, TGF - β) levels of two groups of wounds. Results There was no statistically significant difference in the distribution of wound bacterial strains between the two groups (P>0.05); The wound healing rate of the research group was higher than that of the control group, and the difference was statistically significant (P<0.05); Compared with before treatment, the levels of IL-6 and TNF - α in the two groups of patients decreased significantly, while the levels of VEGF and TGF - β increased significantly, with statistical significance (P<0.05), and the improvement effect in the study group was significantly better than that in the control group. Conclusion PD nano hydrogel has significant efficacy in the treatment of CRW, which can effectively eliminate inflammatory reaction, improve the level of growth factors, promote wound healing, and has good safety. However, its ability to inhibit bacterial growth needs further research and verification.
