| 目的：观察四君子汤合丹参饮加减对慢性萎缩性胃炎（chronic atrophic gastritis,CAG）的疗效及对患者血液流变学与肾素-血管紧张素-醛固酮（Ranin Angiotensin System,RAS）系统的影响。方法：按入排标准选取60例脾虚血瘀型CAG患者，病例无脱落，按随机数字法分为对照组和观察组各30例。对照组口服瑞巴派特片治疗，观察组口服四君子汤合丹参饮加减治疗，连续治疗3个月后， 观察治疗有效率，中医证候积分及胃镜病理积分变化，并检测血液流变学、血管紧张素II与胃粘膜血管紧张素II受体1水平。结果：观察组治疗总有效率86.6%，明显高于总有效率为70%的对照组（P<0.05）。中医症候积分、胃镜病理积分经两组治疗后均下降，且与对照组相比，观察组下降更明显（P<0.05）；关于血流变学、血管紧张素II、胃粘膜血管紧张素II受体1水平，对照组治疗前后无明显差异（P>0.05），观察组治疗后较治疗前均明显降低，且与对照组相比有统计学差异（P<0.05）。结论：四君子汤合丹参饮加减对CAG患者的临床症状有明显改善作用，可能是通过改善患者胃粘膜血液流变学，影响患者的胃粘膜RAS系统，从而改善患者胃粘膜血流，促进胃黏膜修复,延缓胃粘膜萎缩进程。 |

| Abstract:Objective: To observe the efficacy of modified Sijunzi Decoction combined with Danshen Drink on chronic atrophic gastritis(CAG) and its effect on patients' blood rheology and renin angiotensin aldosterone system(RAS). Methods:Sixty patients with spleen deficiency and blood stasis type CAG were selected according to the inclusion criteria, and the cases were not shedding, and were divided into 30 cases each in the control group and the observation group according to the random number method.The control group was treated with Rebapatide tablets, and the observation group was treated with modified Sijunzi Decoction combined with Danshen Drink. After 3 months of treatment, we monitored the changes in clinical efficacy, Chinese medicine syndrome score and gastroscopy pathology score.Additionally,we examined blood rheology, angiotensin II and gastric mucosal angiotensin II receptor 1 levels. Results: Compared to the control group,the observation group had a greater total effective rate of 86.6% (P<0.05). Both the observation group and the control group’s TCM symptom score and gastroscopic pathological score decreased after treatment, and there was a significant difference between this two group (P<0.05).The blood rheology value, angiotensin II, and the gastric mucosal angiotensin II receptor 1 leve were shown to be no significantly lower in the control group between before and after treatment(P>0.05), and they were all significantly lower in the observation group compared with the pre-treatment period (P<0.05), and with a greater degree of decrease than in the control group (P<0.05). Conclusion: Sijunzi Decoction combined with Danshen Drink can markedly alleviate the clinical symptoms of patients with CAG, probably by enhancing blood rheology and affecting the RAS system of the gastric mucosa of the patients, thus improving the blood flow of the gastric mucosa, promoting its repairment,thereby slowing down the processsion of CAG. |
