      目的 探讨不同角度等长收缩伸膝状态下膝周肌群的表面肌电激活特征,为膝关节疾病的康复评估与训练提供临床参考。方法 浙江大学医学院附属邵逸夫医院康复医学科招募15例健康成年受试者纳入研究,受试者保持坐位并在股四头肌训练椅上完成固定膝关节屈曲角度下(90°、60°、30°、0°)的最大等长伸膝动作,在测试过程中采集受试者膝周肌群(股内侧肌、股外侧肌、半腱半膜肌、股二头肌)的表面肌电信号进行分析。结果 与屈膝90°相比,屈膝60°、屈膝30°和屈膝0°动作下股外侧肌和半腱半膜肌AEMG值显著降低,差异均有统计学意义(P<0.05);与屈膝60°相比,屈膝0°动作下股外侧肌和半腱半膜肌AEMG值显著降低,差异均有统计学意义(P<0.05)。与屈膝90°相比,屈膝0°动作下股内侧肌/股外侧肌AEMG值比值显著增高,差异有统计学意义(P<0.05)。与屈膝90°相比,屈膝60°和屈膝0°动作下半腱半膜肌/股二头肌AEMG值比值显著增高,差异有统计学意义(P<0.05)。结论 坐位状态下进行股四头肌固定角度伸膝训练时,随着膝关节屈曲角度减少,股外侧肌肌电激活降低而半腱半膜肌肌电激活增加,股内侧肌/股外侧肌比值和半腱半膜肌/股二头肌比值增加。

      Objective To investigate the surface electromyography (sEMG) activation characteristics of peri-knee muscles during isometric knee extension at different angles, providing clinical insights for rehabilitation assessment and training of knee joint disorders. Methods Fifteen healthy adult subjects were recruited from the Department of Rehabilitation Medicine, Sir Run Run Shaw Hospital. Participants performed maximal isometric knee extensions at fixed knee flexion angles (90°, 60°, 30°, 0°) in a seated position using a quadriceps training chair. sEMG signals from peri-knee muscles (vastus medialis, vastus lateralis, semitendinosus/semimembranosus, and biceps femoris) were recorded and analyzed. Results Compared with 90° knee flexion, the average electromyography (AEMG) values of the vastus lateralis and semitendinosus/semimembranosus significantly decreased at 60°, 30°, and 0° knee flexion angles (P<0.05). Compared with 60° knee flexion, the AEMG values of these muscles further decreased at 0° flexion (P<0.05). The vastus medialis/vastus lateralis (VM/VL) AEMG ratio significantly increased at 0° compared to 90° flexion (P<0.05). The semitendinosus/semimembranosus to biceps femoris (ST+SM/BF) AEMG ratio significantly increased at 60° and 0° compared to 90° flexion (P<0.05). Conclusion During seated isometric knee extension training, as the knee flexion angle decreases, the sEMG activation of the vastus lateralis decreases while that of the semitendinosus/semimembranosus increases. Concurrently, the VM/VL and ST+SM/BF activation ratios rise.
