| 本文系统总结蔡宛如教授运用“肺肠同治”理论辨治慢性阻塞性肺疾病急性加重期（AECOPD）的学术思想。蔡师认为，AECOPD的核心病机为“肺失治节-三焦壅滞-气机逆乱”，提出“宣肺通腑法”以调畅气机、涤痰化瘀，并创制宣肺通腑汤，临床疗效显著。文中结合典型医案，阐释其辨治思路与用药特色，为中西医结合治疗呼吸系统急症提供参考。 |

| This article summarizes Professor Cai Wanru"s clinical experience in the treatment of acute exacerbations of chronic obstructive pulmonary disease (AECOPD) based on the theory of "lung and large intestine are on the outside". In view of the common phlegm and heat in patients with AECOPD, the treatment principle of "simultaneous treatment of the lungs and intestines" was proposed, and the addition and subtraction of Xuanlung Tongfu decoction was proposed to relieve heat and phlegm and relieve asthma in combination with traditional Chinese medicine. Clinical practice shows that this method can effectively alleviate cough and asthma symptoms, shorten the course of the disease, and reduce the frequency of acute attacks, which reflects the diagnosis and treatment characteristics of the same treatment under the guidance of the holistic view of traditional Chinese medicine, and provides new ideas and methods for the treatment of AECOPD in traditional Chinese medicine. |
