      目的 探究基于经皮迷走神经刺激(tVNS)辅助治疗对中风患者的言语康复效果。方法 选取我院2023年4月至2024年10月期间诊治的78例脑卒中后失语症(PSA)患者,按照随机数字表法将其分为两组,每组各39例,对照组采用语言康复训练,试验组加以tVNS治疗,分析比较两组康复疗效、语言功能、交流能力以及脑内多种神经递质功能。结果 干预4周后,试验组总有效率高于对照组(P<0.05);试验组中国康复研究中心汉语标准失语症检查量表(CRRCAE)评分高于对照组,且波士顿诊断性失语症检查法(BDAE)失语程度分级优于对照组(P<0.05);试验组日常生活交流能力检查量表(CADL)评分高于对照组(P<0.05);试验组五羟色胺(5-HT)、去甲肾上腺素(NE)、乙酰胆碱(Ach)功率均高于对照组(P<0.05),两组多巴胺(DA)功率对比差异无统计学意义(P>0.05)。结论 tVNS辅助治疗具有良好康复效果,可有效调节PSA患者脑内5-HT、NE、Ach等多种神经递质功率,改善语言功能,增强语言交流能力。

      Objective To explore the effect of adjuvant therapy based on percutaneous vagal nerve stimulation (tVNS) on speech rehabilitation of stroke patients. Methods Seventy-eight patients with post-stroke aphasia (PSA) diagnosed and treated in our hospital from April 2023 to October 2024 were randomly divided into two groups, 39 patients in each group. The control group received language rehabilitation training and the experimental group was treated with tVNS. The rehabilitation effect, language function, communication ability and various neurotransmitter functions in the brain were analyzed and compared between the two groups. Results After 4 weeks of intervention, the total effective rate of the experimental group was higher than that of the control group (P<0.05). The score of Chinese Standard Aphasia Examination Scale (CRRCAE) in the experimental group was higher than that in the control group, and the aphasia grade of Boston Diagnostic Aphasia Examination (BDAE) was better than that in the control group (P< 0.05). The score of daily life communication ability scale (CADL) in the experimental group was higher than that in the control group (P<0.05). The power of serotonin (5-HT), norepinephrine (NE) and acetylcholine (Ach) in the experimental group were higher than those in the control group (P<0.05), and there was no significant difference in the power of dopamine (DA) between the two groups (P>0.05). Conclusion tVNS adjuvant therapy has a good rehabilitation effect, which can effectively regulate the power of 5-HT, NE, Ach and other neurotransmitters in the brain of PSA patients, improve language function and enhance language communication ability.
