      目的 探讨睡眠障碍对儿童注意力及PASS认知模块的具体影响。方法 回顾性研究。分析2020年7月至2024年9月于杭州师范大学附属医院儿童认知睡眠中心就诊的224例儿童(年龄6~14岁),男157例,女67例,排除干扰因素后,按儿童睡眠障碍量表(SDSC)总分分为高分组(≥49分,n=59)、中间组(40~48分,n=97)和低分组(≤39分,n=68)。通过SDSC评估睡眠障碍严重程度,视听整合连续执行测试(IVA-CPT)评估听觉、视觉及综合注意力,DN:CAS认知评估系统分析PASS认知模块功能,应用Kruskal-Wallis H检验及Bonferroni校正对数据进行统计分析。结果 IVA-CPT测试显示高分组在听觉注意商(P=0.025)、视觉注意商(P=0.035)及综合注意商(P=0.018)得分显著低于低分组;中间组介于高、低分组之间,与其他组无显著差异。DN:CAS评估中各组在计划(P=0.079)、同时性加工(P=0.266)、注意(P=0.270)及继时性加工(P=0.490)四个维度均无统计学差异。结论 睡眠障碍显著影响儿童的听觉、视觉及综合注意力,但对PASS认知模块影响有限,表明需重视儿童睡眠障碍的早期筛查与干预,以减少其对注意力的损害。

      Objective To explore the specific effects of sleep disorders on children's attention and PASS cognitive modules. Methods Retrospective study. Analyze 224 children (age 6~14 years old), 157 males and 67 females, who attended the Children's Cognitive Sleep Center of the Affiliated Hospital of Hangzhou Normal University from July 2020 to September 2024, and were divided into a high group (≥49 points, n=59), an intermediate group (40~48 points, n=97), and a low group (≤39 points, n=68), according to the total score of the Sleep Disturbance Scale for Children(SDSC), after excluding interfering factors. The severity of sleep disorders was assessed by SDSC, auditory, visual and combined attention was assessed by the Integrated Visual and Auditory Continuous Performance Test(IVA-CPT), and the functioning of the PASS cognitive module was analyzed by the Das-Naglieri Cognitive Assessment System(DN:CAS), and the data were statistically analyzed by applying the Kruskal-Wallis H-test and Bonferroni correction. Results The IVA-CPT test showed that the high group scored significantly lower than the low group in auditory attention quotient (p=0.025), visual attention quotient (p=0.035), and integrated attention quotient (p=0.018); the intermediate group was in between the high and low groups, and there was no significant difference with the other groups. The groups in DN:CAS assessment scored significantly lower in planning (P=0.079), simultaneous processing (P=0.266), attention (P=0.270) and secondary processing (P=0.490) dimensions were not statistically different. Conclusion Sleep disorders significantly affect children's auditory, visual, and integrated attention, but have a limited effect on the cognitive module of the PASS, suggesting the need to focus on early screening and intervention for sleep disorders in children to minimize their impairments in attention.
