| 目的 研究热牙胶垂直加压法行C形根管充填对牙髓病和根尖周病患者牙周指数水平的影响。方法 纳入2020年1月到2023年12月期间我院诊治的80例牙髓病、根尖周病患者,按随机数表法进行分组,对照组40例采用冷侧方加压法行C形根管充填治疗,研究组各40例采取热牙胶垂直加压法行C形根管充填治疗。比较两组充填效果、临床疗效、牙周根尖疼痛情况、填充时间、牙周指数水平、对比远期疗效。结果 研究组充填效果优于对照组,临床总有效率高于对照组,牙周根尖疼痛情况(NRS)评分低于对照组,填充时间短于对照组(P＜0.05)。研究组治疗后牙周指数指标[牙周袋探诊深度(PD)、牙龈出血指数(GBI)、牙周附着水平(CAL)、菌斑指数(PLI)]水平均低于同期对照组(P＜0.05)。治疗后6个月,研究组与对照组的根尖周评分均明显降低(P均< 0.05)。治疗后6个月,两组之间在根尖周分级上的差异无统计学差异(P > 0.05)。研究组术后6个月及1年治疗成功率均显著低于对照组(P均< 0.05)。结论 采用热牙胶垂直加压法行C形根管充填效果显著,可有效减轻牙髓病和根尖周病患者牙周根尖疼痛程度,调节牙周指数与牙周状态恢复,且远期疗效较好。 |

| Objective:To investigate the effect of C-shaped root canal filling using the warm vertical compaction technique on periodontal index levels in patients with pulpitis and periapical periodontitis. Methods:A total of 80 patients diagnosed with pulpitis or periapical periodontitis at our hospital between January 2020 and December 2023 were enrolled and randomly assigned into two groups (n = 40 each) using a random number table. The control group received C-shaped root canal filling using the cold lateral compaction technique, while the study group underwent filling with the warm vertical compaction technique. Filling outcomes, clinical efficacy, periodontal and periapical pain (NRS score), filling time, periodontal index levels, and long-term efficacy were compared between the two groups. Results:The study group demonstrated superior filling outcomes, higher overall clinical efficacy, lower NRS pain scores, and shorter filling times compared to the control group (P < 0.05). Post-treatment periodontal indices—including probing depth (PD), gingival bleeding index (GBI), clinical attachment level (CAL), and plaque index (PLI)—were significantly lower in the study group than in the control group (P < 0.05). At 6 months after treatment, periapical scores in both groups were significantly reduced (P < 0.05), but there was no statistically significant difference between the two groups in periapical grading (P > 0.05). The study group showed a significantly higher treatment success rate at both 6 months and 1 year postoperatively compared to the control group (P < 0.05). Conclusion: C-shaped root canal filling using the warm vertical compaction technique yields favorable clinical outcomes, effectively alleviates periodontal and periapical pain in patients with pulpitis and periapical periodontitis, improves periodontal index levels, and promotes periodontal recovery. It also demonstrates better long-term efficacy. |
