      功能性消化不良(FD)与抑郁症(MDD)是高发且常见共病的慢性疾病,二者核心中医病机为“痰气郁结”,且在脑–肠轴紊乱、神经递质失衡、炎症反应、HPA轴异常及肠道菌群失调等方面存在共同病理基础。中医“异病同治”理论强调基于共同病机实施统一治疗策略。半夏厚朴汤作为经典方剂,针对“痰气郁结”证候,广泛应用于胃肠及精神疾病的治疗。临床研究表明,半夏厚朴汤能显著改善FD患者的胃肠症状及焦虑、抑郁状况。其药理机制涉及调节神经递质,抑制炎症反应,调节HPA轴功能,促进脑源性神经营养因子(BDNF)分泌,并改善肠道菌群失衡,实现多靶点、多通路的协同治疗。当前研究存在样本量有限、机制探讨不深入等问题。未来应开展高质量、多中心临床试验,结合网络药理学和多组学技术,进一步阐明半夏厚朴汤的作用机制,推动其在FD与MDD共病治疗中的应用。

      Functional dyspepsia (FD) and major depressive disorder (MDD) are common chronic diseases with a high rate of comorbidity. The core traditional Chinese medicine (TCM) pathogenesis for both conditions is “phlegm and qi stagnation,” and they share common pathological mechanisms including brain-gut axis dysfunction, neurotransmitter imbalance, inflammatory responses, hypothalamic-pituitary-adrenal (HPA) axis abnormalities, and gut microbiota dysbiosis. The TCM theory of "treating different diseases with the same treatment" emphasizes applying unified therapeutic strategies based on shared pathogenesis. Banxia Houpu Tang, a classic TCM formula targeting the “phlegm and qi stagnation” pattern, is widely used in treating gastrointestinal and psychiatric disorders. Clinical studies have shown that Banxia Houpu Tang significantly improves gastrointestinal symptoms and alleviates anxiety and depression in FD patients. Its pharmacological mechanisms involve regulation of neurotransmitters, inhibition of inflammatory responses, modulation of HPA axis function, promotion of brain-derived neurotrophic factor (BDNF) secretion, and restoration of gut microbiota balance, achieving multi-target and multi-pathway synergistic effects. Current research is limited by small sample sizes and insufficient mechanistic exploration. Future studies should focus on high-quality, multi-center clinical trials combined with network pharmacology and multi-omics approaches to further elucidate the mechanisms of Banxia Houpu Tang and promote its application in the treatment of FD and MDD comorbidity.
