电针-中药灌肠整合疗法调控盆腔炎性疾病后遗症慢性盆腔痛SPID-CPP(湿热瘀结证)的临床研究

      目的:探究电针联合中药保留灌肠疗法针对湿热瘀结型盆腔炎性疾病后遗症慢性盆腔痛(SPID-CPP)展现出协同增效作用。方法:于温州市中西医结合医院2024年3月-2025年3月期间收治的SPID-CPP患者中选取60例病例资料,根据随机数字表法分组,观察组30例接受电针+中药灌肠治疗,对照组30例接受中药灌肠治疗,观察综合疗效、治疗前后简明疼痛量表(BPI)、局部体征评分、中医症状量化评分及影像学指标变化。结果:观察组采用电针联合中药保留灌肠的二联疗法在综合疗效方面显著优于单一中药灌肠的对照组(P<0.05)。疼痛改善方面,两组治疗后疼痛强度均显著降低,但观察组在不同治疗节点呈现更明显的阶梯式缓解趋势(P<0.05);体征与中医证候改善上,两组局部体征评分及中医证候积分均较治疗前显著下降,而观察组各时间点评分显著低于对照组(P<0.05);影像学指标变化中,两组盆腔积液深度与炎性包块直径均明显减小,但观察组的改善幅度显著优于对照组(P<0.05)。结论:电针联合中药保留灌肠的二联疗法可有效缓解疼痛,改善症状,疗效持久,作用显著。

      Objective: To investigate the synergistic and enhanced therapeutic effect of electroacupuncture combined with traditional Chinese herbal retention enema for sequelae of pelvic inflammatory disease (SPID-CPP) of damp-heat and blood stasis type. Methods: The medical records of 60 SPID-CPP patients admitted to Wenzhou Integrated Traditional and Western Medicine Hospital between March 2024 and March 2025 were collected. The patients were divide into the observation group (n=30, treated with electroacupuncture and traditional Chinese herbal enema) and the control group (n=30, treated with only traditional Chinese herbal enema) according to random number table methods. The comprehensive therapeutic effect, the changes in the Brief Pain Inventory (BPI), local signs scores, quantitative scores of TCM symptoms, and imaging indicators before and after treatment were observed. Results: In terms of comprehensive therapeutic effect, the observation group treated with the dual therapy of electroacupuncture plus traditional Chinese herbal medicine retention enema of was markedly superior to the control group receiving traditional Chinese herbal enema alone (P < 0.05). Regarding pain relief, both groups showed a significant reduction in pain intensity after treatment; however, the observation group exhibited a more pronounced step-wise alleviation at each assessment point (P < 0.05). For improvements in physical signs and TCM syndromes, local sign scores and TCM syndrome scores declined significantly from baseline in both groups, and scores of the observation group at each time point were significantly lower than those of the control group (P < 0.05). On imaging parameters, the depth of pelvic fluid and the diameter of inflammatory masses decreased appreciably in both groups, the improvement range of the observation group was significantly better than that of the control group (P < 0.05).Conclusions: Electroacupuncture combined with traditional Chinese herbal retention enema can effectively relieve pain and improve symptoms, and the curative effect is lasting and significant.
