| 目的 探讨腰椎手术失败综合征(FBSS)的临床诊治情况,为FBSS的诊断及治疗提供依据。方法 回顾性分析2014年3月至2022年9月联勤保障部队第九〇三医院骨科腰椎术后发生FBSS的患者临床资料,根据治疗方式分为:开放手术组和椎间孔镜组,比较各组患者术后的治疗效果及生活质量。结果 44例患者中,行开放手术治疗者21例,行经皮椎间孔镜手术治疗者23例,术后1个月内,行经皮椎间孔镜手术治疗患者术后的疼痛缓解更明显,ODI、JOA评分及Macnab优良率均明显高于开放手术组(P＜0.05),术后3个月两组患者症状均基本缓解,两组间各项评分无显著差异(P＞0.05)。结论 经皮椎间孔镜技术和开放手术治疗都获得了满意的临床疗效,但在术后早期经皮椎间孔镜手术治疗腰椎手术失败综合征获得更显著的临床疗效。 |

| Objective To explore the clinical diagnosis and treatment of failed lumbar spine surgery syndrome (FBSS), and to provide a basis for the diagnosis and treatment of FBSS. Methods To retrospectively analyze the clinical data of patients who developed FBSS after lumbar spine surgery in the Department of Orthopedics of the No. 903 Hospital of the United Logistics Force from March 2014 to September 2022, and to classify them according to the treatment modality into: open surgery group and percutaneous transforaminal technique group, and to compare the postoperative treatment effect and quality of life of patients in each group. Results Among the 44 patients, 21 patients were treated with open surgery and 23 patients were treated with percutaneous transforaminal technique surgery. The VAS, ODI, JOA scores and Macnab excellence rate of the patients treated with percutaneous transforaminal technique surgery were significantly lower than those of the open surgery group in the postoperative period of 1 month (P < 0.05), and the symptoms of the patients in the two groups basically relieved in the postoperative period of 3 months, and the scores of the patients in the two groups were not significantly different (P > 0.05). Conclusion Both percutaneous transforaminal technique and open surgical treatments have yielded satisfactory clinical outcomes, but more significant clinical outcomes have been obtained in the treatment of lumbar spine surgery failure syndrome by percutaneous transforaminal technique surgery in the early postoperative period. |
