| 目的 本研究通过建立深Ⅱ度烫伤大鼠模型,评估复方血竭烧烫伤药膏的药效学作用,并阐明其作用机制。 方法 36只6-8周龄SD大鼠适应性饲养7天后,按性别与体重随机分为空白组、模型组、复方血竭烧烫伤药膏(0.1 g、0.2 g、0.4 g/只)组和京万红软膏(0.2 g/只)组,每组6只,雌雄各半。除空白组外将大鼠背部皮肤浸入80°C水浴15秒建立深Ⅱ度烫伤模型,各治疗组每日外敷相应药物,连续21天。造模后第2、7、14和21天,采用透明图纸描绘并称重法测定创面面积,计算愈合率;造模后24和48小时,ELISA法检测血清TNF-α及IL-1β水平;造模后第21天,取烫伤部位皮肤组织,HE染色后观察皮肤病理学变化,并采用RNA测序(RNA-seq)法研究分子机制。结果 与模型组相比,复方血竭烧烫伤药膏(0.1 g、0.2 g、0.4 g/只)呈剂量依赖性显著提高第21天创面愈合率(87.3 ± 2.3%、91.7 ± 3.3%、95.4 ± 3.9%,P = 0.027、0.007、0.002)。ELISA检测显示,药膏各剂量组均显著降低烫伤后24 h和48 h血清TNF-α及IL-1β水平(P < 0.05)。病理学分析显示,其显著改善皮肤损伤并减少炎症细胞浸润(病理评分分别为3.0 ± 0.9、2.0 ± 0.6、1.7 ± 0.8,P = 0.035、< 0.001、< 0.001)。与阳性对照京万红软膏相比,复方血竭烧烫伤药膏在相同给药剂量下的皮肤愈合能力及抗炎效果差异无统计学意义(P > 0.05),但其病理学评分呈降低趋势。RNA-seq分析表明,复方血竭烧烫伤药膏通过抑制NF-κB信号通路及Th1/Th2细胞分化通路、下调促炎基因表达,同时上调与皮肤修复相关的GDF1和角蛋白家族基因,减轻皮肤损伤。 结论 复方血竭烧烫伤药膏可在炎症早期抑制血清促炎细胞因子升高,并在修复期促进皮肤结构恢复,具有显著抗炎和促愈合作用,是一种具有潜力的新型烧烫伤治疗药物。 |

| Objective This study aimed to evaluate the pharmacodynamic effects and elucidate the underlying mechanisms of a compound sanguis draxonis burn ointment in a rat model of deep second-degree scald injury. Methods Thirty-six 6–8-week-old Sprague–Dawley rats of both sexes were acclimatized for 7 days and then randomly assigned to six groups: control, model, compound Sanguis draconis burn ointment (0.1 g, 0.2 g, and 0.4 g/rat), and Jingwanhong ointment (0.2 g/rat) (n = 6 per group). Except for the control group, a deep second-degree burn model was established by immersing the dorsal skin of rats in an 80 °C water bath for 15 s. The corresponding ointments were topically applied once daily for 21 days. Wound areas were traced on transparent paper and quantified using a weight-based planimetric method to calculate wound-healing rates on days 2, 7, 14, and 21 post-injury. Serum levels of tumor necrosis factor-α (TNF-α) and interleukin-1β (IL-1β) were measured at 24 h and 48 h after injury by ELISA. On day 21, skin tissues from the scalded area were collected for histopathological examination using hematoxylin–eosin staining, and RNA sequencing (RNA-seq) was performed to investigate the molecular mechanisms involved. Results Compared with the model group, the ointment groups exhibited a dose-dependent and significant increase in wound healing rates on day 21 (87.3 ± 2.3%, 91.7 ± 3.3%, and 95.4 ± 3.9% for 0.1 g, 0.2 g, and 0.4 g, respectively; P = 0.027, 0.007, 0.002). ELISA results indicated that all ointment doses significantly reduced serum TNF-α and IL-1β levels at both 24 h and 48 h post-burn (P < 0.05). Histological analysis showed marked improvement in skin damage and reduced inflammatory cell infiltration (pathological scores: 3.0 ± 0.9, 2.0 ± 0.6, and 1.7 ± 0.8; P = 0.035, < 0.001, < 0.001). Compared with the positive control Jingwanhong ointment, the compound sanguis draxonis burn ointment exhibited comparable skin wound-healing capacity and anti-inflammatory efficacy at the same dosage, with no statistically significant difference (P > 0.05); however, the histopathological scores showed a decreasing trend. RNA-seq analysis revealed that the ointment attenuated skin injury by inhibiting NF-κB signaling and Th1/Th2 cell differentiation pathways, downregulating pro-inflammatory genes, and upregulating growth differentiation factor 1 (GDF1) and keratin family genes associated with skin repair. Conclusion The compound Sanguis draconis burn ointment can suppress early systemic inflammatory cytokine elevation and promote structural restoration of the skin during the repair phase. It exhibits significant anti-inflammatory and wound-healing effects and shows potential as a novel therapeutic for burn and scald injuries. |
