      慢性阻塞性肺疾病(COPD)以持续性气流受限和慢性气道炎症为特征,是全球重大公共卫生问题。“肺与大肠相表里”理论与现代“肠-肺轴”学说为COPD防治提供了新视角。灵芝具有扶正固本之功,其多糖、三萜等活性成分可抗炎、免疫调节并重塑肠道微生态。本文综述近五年研究,系统阐述灵芝通过重塑肠道菌群、促进短链脂肪酸(SCFAs)生成、抑制核因子-κB(NF-κB)信号通路以减轻COPD肺部炎症的潜在机制。现有证据表明,灵芝调节“肠道菌群-SCFAs-免疫炎症轴”的作用,可为阐释“肺与大肠相表里”科学内涵及开发靶向肠-肺轴的COPD治疗策略提供理论依据。
英文摘要:
      Chronic Obstructive Pulmonary Disease (COPD) is a major global health issue characterized by persistent airflow limitation and chronic airway inflammation. The “lung and large intestine being interior-exteriorly related” theory and the “gut-lung axis” concept provide novel insights into COPD management. Ganoderma lucidum, a key herb for reinforcing vital qi, contains polysaccharides and triterpenoids with anti-inflammatory, immunomodulatory, and gut microbiota-regulating properties. This review summarizes research progress over the past five years, elucidating the potential mechanisms by which Ganoderma lucidum alleviates pulmonary inflammation in COPD, including remodeling the gut microbiota, promoting short-chain fatty acid (SCFA) production, and inhibiting the nuclear factor-kappa B (NF-κB) pathway. Current evidence suggests that Ganoderma lucidum modulates the “gut microbiota-SCFAs-immune inflammation” axis, providing a theoretical basis for interpreting the scientific connotation of the “exterior-interior relationship between lung and large intestine” and for developing gut-lung axis-targeted interventions for COPD.
