      目的:探究茵陈退黄健脾方药浴结合阳陵泉按揉治疗新生儿黄疸(NNJ)(湿热郁蒸证)的临床疗效及对氧化应激、炎症反应的影响。方法:选取2023年1月-2025年7月平顶山市中医医院收治的NNJ患儿154例,采用随机数字表法分为观察组和对照组各77例。对照组采用常规蓝光照射治疗联合阳陵泉穴按揉,观察组在对照组基础上进行茵陈退黄健脾方药浴治疗,观察两组患儿临床情况、血清胆红素水平、氧化应激及炎症水平,记录两组治疗过程中不良反应发生情况。结果:观察组疗效高于对照组(P<0.05),黄疸消退时间、大便转黄时间、光疗时间以及住院天数均短于对照组(P<0.05);治疗前两组总胆红素(TBIL)、直接胆红素(DBIL)、间接胆红素(IBIL)、丙二醛(MDA)、C反应蛋白(CRP)、白介素-6(IL-6)、超氧化物歧化酶(SOD)水平均无统计学意义(P>0.05),治疗后,观察组TBIL、DBIL、IBIL水平均低于对照组(P<0.05),治疗后,两组MDA、CRP、IL-6均降低,且观察组低于对照组(P<0.05),治疗后两组SOD均低于治疗前,但观察组治疗后SOD水平高于对照组(P<0.05);两组不良反应发生率比较差无统计学意义(P>0.05)。结论:茵陈退黄健脾方药浴结合阳陵泉按揉治疗NNJ(湿热郁蒸证)疗效显著,有效改善患儿临床症状,降低血清胆红素水平,减轻氧化应激损伤和炎症反应,且安全性较好,为新生儿黄疸的临床治疗提供一种有效的中西医结合方案。

      Objective: To investigate the clinical efficacy of a medicinal bath formulation containing Yinchen Tuihuang Jianspi combined with massage of the Yanglingquan acupoint in the treatment of neonatal jaundice (NNJ) (characterised by the pattern of damp-heat stagnation), and its effects on oxidative stress and inflammatory responses. Methods: A total of 154 paediatric patients with neonatal jaundice (NNJ) admitted to Pingdingshan Hospital of Traditional Chinese Medicine between January 2023 and July 2025 were selected and randomised using a random number table into an observation group and a control group, each comprising 77 patients. The control group received conventional blue light phototherapy combined with massage of the Yanglingquan acupoint, whilst the observation group received the same treatment as the control group supplemented with herbal baths using the Yinchen Tuihuang Jianspi Formula. Clinical status, serum bilirubin levels, oxidative stress and inflammatory markers were assessed in both groups, and the incidence of adverse reactions during treatment was recorded. Results: The treatment efficacy in the observation group was higher than that in the control group (P < 0.05); the time to resolution of jaundice, the time until stools turned yellow, the duration of phototherapy, and the length of hospital stay were all shorter in the observation group than in the control group (P < 0.05); Prior to treatment, there were no statistically significant differences between the two groups in total bilirubin (TBIL), direct bilirubin (DBIL), indirect bilirubin (IBIL), malondialdehyde (MDA), C-reactive protein (CRP), interleukin-6 (IL-6) or superoxide dismutase (SOD) levels (P > 0.05); post-treatment, the levels of TBIL, DBIL and IBIL in the observation group were lower than those in the control group (P < 0.05); post-treatment, MDA, CRP and IL-6 levels decreased in both groups, with the observation group showing lower levels than the control group (P < 0.05); post-treatment, SOD levels in both groups were lower than pre-treatment levels, but the observation group had higher post-treatment SOD levels than the control group (P < 0.05); There was no statistically significant difference in the incidence of adverse reactions between the two groups (P > 0.05). Conclusion: The combination of herbal bath therapy using the Yinchen Tuihuang Jianspi Formula and massage of the Yanglingquan acupoint demonstrates significant efficacy in the treatment of neonatal jaundice (damp-heat stagnation syndrome). It effectively improves clinical symptoms in infants, reduces serum bilirubin levels, and alleviates oxidative stress damage and inflammatory responses, whilst exhibiting good safety. This approach provides an effective integrated traditional Chinese and Western medicine treatment strategy for neonatal jaundice.
