中医药靶向骨肉瘤糖酵解重编程的研究进展

      骨肉瘤是最常见的原发性恶性骨肿瘤之一,具有侵袭性强、易复发转移及预后差等特点。尽管新辅助化疗联合手术切除等综合治疗模式已在一定程度上改善患者预后,但复发、转移及化疗耐药仍是当前临床治疗面临的主要难题。近年来,肿瘤代谢重编程逐渐成为骨肉瘤研究的重要热点,其中糖酵解重编程被认为是维持骨肉瘤细胞快速增殖、侵袭转移、干性维持及治疗抵抗的重要代谢基础。中医药因具有多成分、多靶点和多通路协同调控的特点,在骨肉瘤防治中显示出一定优势,部分研究表明其可通过调控糖酵解相关关键酶、磷脂酰肌醇3-激酶/蛋白激酶B(phosphatidylinositol 3-kinase/protein kinase B,PI3K/AKT)信号轴及其他代谢相关网络,抑制骨肉瘤细胞增殖、迁移和侵袭,并诱导凋亡、铁死亡等过程。本文围绕骨肉瘤糖酵解重编程的生物学基础、中医药靶向糖酵解重编程干预骨肉瘤的研究进展以及该策略的优势与局限进行综述,以期为阐明中医药抗骨肉瘤的代谢机制、拓展骨肉瘤代谢靶向治疗思路及推动相关研究进一步深入提供参考。

      Osteosarcoma is one of the most common primary malignant bone tumors and is characterized by strong invasiveness, a high tendency for recurrence and metastasis, and poor prognosis. Although comprehensive treatment strategies based on neoadjuvant chemotherapy combined with surgical resection have improved patient outcomes to some extent, recurrence, metastasis, and chemoresistance remain major challenges in current clinical management. In recent years, metabolic reprogramming has gradually become an important focus in osteosarcoma research, among which glycolytic reprogramming is considered a crucial metabolic basis for the rapid proliferation, invasion, metastasis, stemness maintenance, and therapeutic resistance of osteosarcoma cells. Traditional Chinese medicine, characterized by multi-component, multi-target, and multi-pathway synergistic regulation, has shown certain advantages in the prevention and treatment of osteosarcoma. Some studies have shown that it can inhibit the proliferation, migration, and invasion of osteosarcoma cells and induce apoptosis, ferroptosis, and other processes by regulating glycolysis-related key enzymes, the PI3K/AKT signaling axis, and other metabolism-related networks. This article reviews the biological basis of glycolytic reprogramming in osteosarcoma, the research progress of traditional Chinese medicine targeting glycolytic reprogramming in osteosarcoma, as well as the advantages and limitations of this strategy, with the aim of providing references for clarifying the metabolic mechanisms of traditional Chinese medicine against osteosarcoma, expanding the therapeutic strategies targeting metabolism in osteosarcoma, and promoting further research in this field.
