| 目的 探究分析淋巴细胞间质性肺炎患者临床诊断过程中,多层螺旋CT的诊断价值。方法 选取我院呼吸科2013年6月～2016年6月初步诊断为淋巴细胞间质性肺炎可疑的52例患者为研究对象,采用自身配对盲法设计试验,对每位患者分别用普通CT与多层螺旋CT做进一步诊断,最后经病理检查进行确诊,以病理学检查确诊结果为金标准,对两种诊断结果进行综合评价,并绘制两种方法的 ROC曲线下面积(AUG),对比分析两者的诊断价值。结果 经病理学检查最终诊断为淋巴细胞间质性肺炎的患者共35例；多层螺旋CT诊断组较普通CT组具有较高的灵敏度和特异度,且漏诊率与误诊率较低；其ROC曲线下面积(AUG=0.916)也明显高于普通CT组(AUG=0.761),P<0.05。结论 多层螺旋CT在淋巴细胞间质性肺炎诊断效果优于普通CT诊断,可为临床诊断提供更有价值的参考,单纯的多层螺旋CT检查仍然存在少量的误诊与漏诊病例,临床上需要联合其他诊断手段进行确诊,以进一步提高其诊断价值。 |

| Objective To investigate the diagnostic value of high resolution CT in the clinical diagnosis of lymphocytic interstitial pneumonia. Methods In our hospital department of respiration from June 2013 to early June 2016 in diagnosis of lymphocytic interstitial pneumonia suspected in 52 patients as the research object, using self-paired design test blind method, for each patient using general CT and multi-slice spiral CT for further diagnosis, finally diagnosed by pathological examination. The pathological examination confirmed the diagnosis of gold standard two, to evaluate the diagnostic results, and draw the ROC curve area under the two methods (AUG), comparative analysis of diagnostic value of the two. Results Take pathological examination, the final diagnosis of lymphocytic interstitial pneumonia patients in 35 cases; multi slice spiral CT diagnosis group than in the normal group CT with high sensitivity and specificity, and the rate of missed diagnosis and misdiagnosis rate is low; the area under the ROC curve (AUG=0.916) were significantly higher than those in normal CT group (AUG=0.761), two there was a significant difference between group AUG (P<0.05). Conclusion The effect of diagnosis of multi-slice spiral CT is superior to the conventional CT in diagnosis, can provide valuable reference for clinical diagnosis, but the multi-slice spiral CT, there are still a few cases of misdiagnosis and missed diagnosis, the clinical need in combination with other diagnostic methods for diagnosis, in order to further improve its diagnostic value. |
