| 【】 目的 探讨应用前外侧入路治疗累及后外侧区的胫骨平台骨折的短期疗效。 方法 收集2014年3月至2016年3月应用前外侧入路联合排筏螺钉技术治疗累及后外侧区的胫骨平台骨折的11例患者资料,男7例,女4例；年龄32~61岁,平均45岁。采用Rasmussen放射学评分对骨折复位情况进行评估；采用美国特种外科医院(hospital for special surgery,HSS)膝关节功能评分进行膝关节功能评估。结果 11例患者手术时间为71~240分钟,平均131分钟；骨折愈合时间10~16周,平均13周。术后Rasmussen放射学评分为12~18分,平均15.2分,其中优7例,良4例,优良率100%。末次随访HSS膝关节评分平均为89分(83～97分),功能评分均为优。 结论 前外侧入路联合排筏螺钉技术能够处理大部分的累及后外侧的胫骨平台骨折,具有手术时间短、固定牢靠、功能恢复好、并发症少的优点,适合临床广泛应用。 |

| 【】Object To investigate the effect of treating tibial plateau fractures involving the posterolateral column via the anterolateral approach. Methods From Mar 2014 to Mar 2016,11 patients with tibial plateau fractures involving the posterolateral column were treated through the anterolateral approach by proximal tibial plates with 4 raft roles. The group included 7males and 4 females, aged from 32 to 61 years old(average 45 years old). All patients’ curative effect was estimated by using Rasmussen radiological score and HSS grading. Results The operation time of 11 patients was 71 ~ 240 minutes, average 131 minutes. Fracture union time was 10 ~ 16 weeks, average 13 weeks. The postoperative radiological score of Rasmussen was 12 ~ 18 points, averaging 15.2 points, of which 7 cases were excellent, 4 cases were good, and the optimal rate was 100%. The average score of HSS was 89 points (83 ~ 97 points). Conclusions The anterolateral approach by proximal tibial plates with 4 raft roles to treat tibial plateau fractures involving the posterolateral column is an efftct method. Short operation time, reliable fixation, good functional recovery and less complication were observed. |
