      目的 观察绝经后女性雌激素水平、血型及血脂水平与冠心病的变化,探讨其可能的临床意义。方法 纳入359例绝经后女性,根据患者临床症状及冠状动脉CTA、冠状动脉造影检查结果分为冠心病组179例及对照组180例。两组分别在固定时间下检测血浆总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)水平、血型、性激素五项等。结果 与对照组比较,冠心病组的FSH、TC、HDL-C增高(p<0.01),E2、HDL-C下降明显(p<0.01)。冠心病组的 ABO 血型分布为 A > B> O >AB,与对照组为 O > B>A > AB 。冠心病组和对照组组间O 型与A 型分布存在显著差异(p<0.05)。经过Pearson相关分析显示,E2与TC、LDL-C、A型血与冠心病发生呈负相关性(p<0.01),与HDL、O型血呈正相关。结论 绝经后女性体内E2水平、血型与LDL-C水平一样同为女性冠心病的独立危险因素。

      ABSTRACT Objective To investigate the changes of estrogen levels, blood type and blood lipid levels in postmenopausal women with coronary heart disease (CHD), and to explore its possible clinical significance. Methods Three hundred and fifty-nine postmenopausal women were enrolled in this study. According to the clinical symptoms, coronary CTA and coronary angiography findings, Three hundred and fifty-nine patients were divided into coronary artery disease group (179 cases) and control group (180 cases). The two groups were detected in plasma total cholesterol in the fixed time (TC), triglyceride (TG), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C) level, blood type, five sex hormones etc.. Results Compared with the control group, the FSH, TC and HDL-C in CHD group increased (p<0.01), and E2 and HDL-C decreased significantly (p<0.01). The ABO blood group distribution in coronary heart disease group was A > B > O > AB, and O < B > A > AB in the control group. There was significant difference between O group and A group in the coronary heart disease group and the control group (p<0.05). Pearson correlation analysis showed that there was a negative correlation between E2 and TC, LDL-C, A type blood and coronary heart disease (p<0.01), and positively correlated with HDL and O type blood. Conclusions The levels of E2 and blood type in postmenopausal women are similar to those of LDL-C, and they are independent risk factors of CHD in women.
