      目的 探讨程序性死亡配体1(PD-L1)和细胞毒性T淋巴细胞相关蛋白4(CTLA-4)在肺癌合并肺结核中的免疫组化表达情况及其与临床病理特征的相关性。方法 收集杭州市红十字会医院胸外科及结核外科收治的96例病例,其中肺癌合并肺结核组患者38例、单纯肺结核患者29例(结核组)和单纯肺癌患者29例(肺癌组),采用免疫组化方法检测各组患者标本PD-L1、CTLA-4的表达水平,分析其表达情况和与临床病理特征的相关性。结果 CTLA-4在肺癌合并肺结核组中的阳性表达率高于结核组和肺癌组,表达分别为65.8%、24.1%、20.7%;PD-L1在肺癌组、肺癌合并肺结核组的肿瘤细胞的表达率分别为6.9%、10.5%,PD-L1在结核组、肺癌组和肺癌合并肺结核组间质中的表达率分别为48.3%、31%、39.5%。在肺癌合并肺结核组中,CTLA-4在鳞癌、低中分化癌、肿瘤最大径≥2.7cm的病例中阳性表达率更高(均P<0.05)。且CTLA-4与肿瘤类型、肿瘤大小有显著相关性。结论 在肺癌合并肺结核的病例中,CTLA-4、PD-L1均存在表达,且CTLA-4的表达与临床病理特征密切相关,在肺结核和肺癌的免疫中都发挥了负性调节的作用,这两者可能在疾病的发展过程中存在协同作用,与肺结核促进肺癌的发生发展有密切关联。

      Objective To investigate the immunohistochemical expression of programmed death ligand 1 (PD-L1) and cytotoxic T-lymphocyte-associated protein 4 (CTLA-4) in lung cancer combined with tuberculosis and its correlation with clinicopathological features. Methods Ninety-six cases admitted to the Department of Thoracic Surgery and Department of Tuberculosis Surgery of Hangzhou Red Cross Hospital were collected, including 38 patients in the group of lung cancer combined with tuberculosis, 29 patients with simple tuberculosis (tuberculosis group), and 29 patients with simple lung cancer (lung cancer group), and immunohistochemistry was used to detect the expression levels of PD-L1 and CTLA-4 in specimens of patients in the various groups, and to analyze the expression and correlation with the clinicopathological features. . Results The positive expression rate of CTLA-4 in the lung cancer combined with tuberculosis group was higher than that in the tuberculosis group and the lung cancer group, with the expression of 65.8%, 24.1%, and 20.7%, respectively; the expression rate of PD-L1 in the tumor cells in the lung cancer group and the lung cancer combined with tuberculosis group was 6.9% and 10.5%, and the expression rate of PD-L1 in the interstitium of the tuberculosis group, the lung cancer group, and the lung cancer combined with tuberculosis group was 48.3%, 31%, and 39.5%. In the lung cancer combined with tuberculosis group, the positive expression rate of CTLA-4 was higher in squamous carcinoma, low to moderately differentiated carcinoma, and cases with the largest diameter of the tumor ≥2.7 cm (all P<0.05). And there was a significant correlation between CTLA-4 and tumor type and tumor size. Conclusion In cases of lung cancer combined with tuberculosis, CTLA-4 and PD-L1 were expressed, and the expression of CTLA-4 was closely related to the clinicopathological features, and played a negative regulatory role in the immunity of both tuberculosis and lung cancer, which may have a synergistic effect in the process of the disease, and are closely related to the promotion of the development of lung cancer by tuberculosis.
