      [摘要] 目的 探讨血清淀粉酶样蛋白A/白蛋白比值(SAR)与冠脉病变程度的相关性。方法:选取2022年1月至2023年1月于杭州市中医院行冠状动脉造影患者276例。根据造影结果分为冠心病组(191例)和正常组(85例)。依据冠脉Gensini评分将冠心病患者分为冠状动脉轻度病变组(GS<38,n=94)及冠状动脉严重病变组(GS≥38,n=97)。记录患者的一般临床资料、各组的血清SAA、ALB水平,并计算SAR。比较两组及冠心病亚组患者的ALB、SAA、SAR等指标;采用回归分析筛选冠心病及冠脉严重病变的危险因素;ROC曲线分析探讨SAR对冠脉严重病变的预测价值。结果 SAR与Gensini评分呈正相关;CHD组的SAR水平高于正常组,冠脉严重病变组的SAR高于轻度病变组(P<0.01);SAR为冠心病及冠状动脉严重病变的独立危险因素(P<0.05);SAR的诊断价值高于SAA、ALB,其AUC为0.701(P<0.01),最佳诊断阈值为0.17,敏感度为0.734,特异度为0.619。结论 CHD患者血清SAR对冠心病及冠脉严重病变有较高的预测价值,且优于SAA、ALB,为临床诊断、治疗及预后提供参考。

      [Abstract] Objective To search the link between Serum amyloid A-to-albumin rato(SAR) and the severity of coronary artery occlusion. Methods Selecting 276 patients who performed coronary disease at Hangzhou Traditional Chinese Medicine Hospital from January 2022 to January 2023.Angiography results split into two factions that the normal (85 cases) and the coronary heart disease (191 cases) .Coronary heart disease patients were separated into mild coronary artery lession group (GS<38, n=94) and severe coronary artery lession group (GS≥38, n=97) based on the coronary Gensini scores. Record the general clinical data of patients, serum levels of SAA and ALB in each group, and calculate SAR. Compare the ALB, SAA, SAR and other indicators between the two groups and the subgroup of patients with coronary heart disease;Using regression analysis to sift the risk factors of CHD and severe coronary artery disease.Exploring the predictive value of SAR for severe coronary artery disease through ROC curve analysis.Results SAR is positive correlation with Gensini score;The CHD group's SAR surpassed that of the normal group and the severe coronary artery disease group's SAR was greater than that of the moderate disease group (P<0.01); The independent hazard factor of SAR for CHD and severe coronary artery occlusion was evident.SAR was an independent hazard factor for CHD and severe coronary artery occlusion (P<0.05); SAR's diagnostic value surpassed that of SAA and ALB, with an AUC of 0.701 (P<0.01), an optimal diagnostic threshold of 0.1711, a sensitivity of 0.734, and a specificity of 0.619. Conclusion Higher SAR has a remarkable prognostic valuer for CHD and serious coronary artery disease, surpassing SAA and ALB in terms of clinical diagnosis, therapy, and prognosis.
